What is form EX-18?
Form EX-18 is the application for the EU Citizen Registration Certificate, the document that certifies the right of residence in Spain for citizens of the EU, EEA, and Switzerland.

Form EX-18: Central Register of Foreign Nationals (EU)
Form EX-18 is the application for the EU Citizen Registration Certificate, mandatory for citizens of the EU, EEA, and Switzerland who reside in Spain for more than three months. It lets you obtain your NIE and officially register in Spain.
Complete the EX-18 with your personal details, the activity that justifies your residence (work, studies, sufficient financial means), your health insurance, and proof of fee payment. Download it as a PDF and submit it at your appointment.
After 5 years of legal, continuous residence in Spain, you can apply for the permanent residence certificate using the same form by checking the corresponding box. Always keep your certificate up to date if you change your address.
